Community Home Care is a team of caring professionals providing health care services in the home setting for Elmore, Tallapoosa, and Macon county residents. Home Care addresses the physical, social and emotional needs of clients recovering from deliberating illness or injury. Services include Nursing, Physical Therapy, Speech Therapy, and Occupational Therapy, Personal Care, and Medical Social Services. The services, directed by your physician, are provided to persons who are essentially homebound, requiring the skill of a professional nurse or therapist and delivered on an intermittent basis.
